If we are out of network with your insurer, you may be responsible for higher deductibles, copays, and/or co-insurance. You may be considered self-pay if we are out of network or if you are unable to provide proof of insurance. A deposit may be required before surgery. This amount will be an estimate based on your insurance policy(s) and is subject to change. Hospital, anesthesia, pathology and any other facility fees will be billed separately.
